Before Your Appointment!

We would like to welcome you as a patient to Northeastern Rural Health Clinics and would like to make your first appointment as easy as possible. Please feel free to review and print any of the forms to the right. We also have these forms available in the office but please make sure to allow time to fill out our New Patient Packet before your appointment if you do not bring it with you.

Also, please remember to bring your co-payment, photo ID and insurance card to every appointment. You may need to be rescheduled if you do not bring your copay at time of visit.

  • Review our No Show Policy and our Payment Policy at the back of the Patient Packet

We also offer sliding fee discounts to patients who qualify. All co-pays and sliding fee fees are due at time of service please bring your co-pay with you to your appointment. What is PCMH?

Patient-Centered Medical Home (PCMH) is an innovative program for improving primary care by focusing on the patient. Working in teams, coordinating care and tracking patient outcomes gives the caregivers a common goal; Patient-Centered Care.

Striving to create quality relationships between the clinician and patient is an integral part of the program. Early evidence suggests that PCMH improves quality and returns savings.
